Open Question: Is consciousness theoretically transferable?

Now, I know nothing about psychology or neuroscience, but if, say, you were able "copy" your mind to another brain or upload it into a computer, is there any possibility that you stream of consciousness would not stop on one end? Meaning, do you think it would be possible to switch brains while retaining the same unaltered, continuous consciousness, your experience of the universe not ending? Vis-à-vis, "you" wouldn't die. I'm guessing I'm explaining this poorly, but please, any opinions would be appreciated.
